First off, the rear sub frame and diff needs to be removed from the car. Refer to the factory service manual on how to do that, or look up various corners of the interwebs, forums, and or Google. Once out, continue to the next step.

 

 Now the old bushing material needs to come out. Get is out by all means necessary without damaging the sub frame too much. You can drill, press, burn, chew, laser beam them out. Just get them out. Its already dead and falling apart, it shouldn't me too hard :)
 Now you have to remove the metal housing for the bushing that is pressed into the subframe. This can be done by taking a Sawzall, or for the people out there with enduring arms, small hacksaw and cut the ring to allow it to compress and bit and come out.
Now take a hammer and go to town. It should come out. If not, keep going till it either A-Comes out, or B-Your arm falls off. In the end, you'll have these.
Now STOP!!, hammer time!...

If you spend more time making sure they go in perfectly strait, with small little taps to get it in strait, use a little lube, it should go in pretty easy. Use something between the hammer and bushing, like a piece of 2X4 or block of aluminum, something so you won't mar the bushing.

Keep going till they are down and flush in the sub frame. Here are pics of one fully seated.
